Certain Waivers under HIPAA. (a) Patient acknowledges that neither Group nor Physician guarantees that communications with Physician using electronic mail ("e-mail"), facsimile, video chat, instant messaging, and cellular telephone are secure or confidential methods of communications. Accordingly, Patient expressly waives Group’s and Physician’s obligations under the Health Insurance Portability and Accountability Act of 1996 (42 U.S.C. § 1320d et seq.), as amended by the Health Information Technology for Economic and Clinical Health Act of 2009, and all rules and regulations promulgated thereunder (collectively, "HIPAA"), and other state and federal laws and regulations applicable to the use, maintenance, and disclosure of patient-related information, to guarantee confidentiality with respect to correspondence using such means of communication. Patient acknowledges that all such communications may become a part of Patient’s medical records maintained by Physician. (b) By providing Patient’s e-mail address to Physician, Patient authorizes Physician to communicate with Patient by e-mail regarding Patient’s "protected health information" ("PHI") (as defined under HIPAA) and Patient understands and agrees to the following: E-mail is not necessarily a secure medium for sending or receiving PHI and, accordingly, any third party may gain access to such PHI; Although Group and Physician will make all reasonable efforts to keep e-mail communications confidential and secure, neither Group nor Physician can assure or guarantee the absolute confidentiality of such e-mail communications.
Patient acknowledges and agrees that Valhalla Vitality, along with their assigns, will be entitled to use any data, discoveries, results, improvements or other information resulting from the Services for any lawful purpose whatsoever, including, but not limited to, internal research, academic or other publications or commercial purposes. All data will be kept on a Cloud Based system that is password protected, and accessible to Valhalla Vitality staff.
I acknowledge that Valhalla Vitality charges for concierge medical services and that, if I choose to use Valhalla Vitality's preferred pharmacy, the cost of medication is included in the price of treatment. All sales are final, unless I am found to be medically ineligible to receive a therapy by my Valhalla Vitality clinician.
I understand that Valhalla Vitality will be sending my medication prescription to a compounding pharmacy that is licensed in my state of residence. The typical manufacturing and processing time for compounded medications is at least 10 business days, although it may take longer and there is no timeline guarantees. As my concierge medical provider Valhalla Vitality will stand by me in making sure that the pharmacy we choose is held to the highest possible customer service and quality standards in fulfillment of my order.
We are committed to providing you with the highest standard of care, including ensuring your convenience and choice in the dispensing of prescriptions. As part of our commitment to patient-centered healthcare, please be informed that you are entitled to have your prescriptions dispensed following your professional consultation directly to your personal or local pharmacies if you so decide. However, we are only able to offer you negotiated and reduced rates for treatments that are manufactured and adequately stocked by our select, partner compounding pharmacies. Your team at Valhalla Vitality works closely with our pharmacy partners to ensure that your prescriptions are transmitted accurately, efficiently, and securely.

While Valhalla Vitality endeavors to furnish accurate information about these treatments, it expressly disclaims any guarantees regarding their effectiveness or suitability for your individual needs. Prior to beginning any treatment plan, individuals must consult with a licensed healthcare provider capable of assessing their unique medical needs and delivering appropriate guidance and care. By engaging in any treatments or therapies offered herein, users expressly acknowledge and agree to assume all associated risks. Before undergoing Wellness Therapy, it is important that you know and understand some general information including the risks and benefits.
Wellness Therapy with Valhalla Vitality is the use of peptides, medications and/or vitamins to improve wellbeing, quality of life and relieve symptoms that are not associated with a particular pathology/disease. A combination of treatments may be used to achieve symptom relief, however if pathology/disease is suspected patients will be referred to their primary physician for diagnosis and treatment. When undergoing wellness therapy with Valhalla Vitality, it is of utmost importance to not modify any ongoing medical treatments prescribed and managed by other medical care providers.
After submitting you patient intake the Valhalla Vitality the medical team will review your medical history and symptoms to determine whether you may be a candidate for WelnessTherapy. Inclusion and exclusion for candidacy is based on presentation of symptoms, current and past medical conditions and currently prescribed medications. If it is determined that you may benefit from Wellness Therapy and do not meet any of our exclusion criteria, a follow up consultation will be conducted and lab tests may be ordered. Consultations are meant to be discrete and convenient for the client, while allowing for all questions to be answered regarding treatment potential benefits, safety, side effects, outcomes and alternatives. If the Valhalla Vitality provider and you as the client both agree that WellnessTherapy is the right choice for you, a prescription will be sent to a licensed US based pharmacy that will mail your medication to your home. After 3 months of treatment, the treatment and progress is reevaluated. At any point as a client and patient of Valhalla Vitality our medical team is openly available to help you with any inquiries, issues and updates regarding your treatment.
Medications prescribed for wellness therapy are often well tolerated, however side effects specific to each medication are possible. There is always the potential for serious side effects and any concern for a serious side effect or allergic reaction to any medication used should be reported at the nearest emergency room. Any potential side effects that you may be experiencing should be presented and discussed with your Valhalla Vitality provider.
Potential benefits of Wellness Therapy are relief of symptoms that improve quality of life.
Wellness Therapy should not be used if you have a history of cancer, untreated clotting disorders, uncontrolled and/or unmonitored high blood pressure, heart dysthymias or severe allergic reactions. Alcohol and other medications should be used with caution as many therapies have the potential to increase or decrease metabolism.
