In regard to Reta, the phase 2 double-blind randomized placebo-controlled trial was done where it included patients with a BMI higher than 30 and BMI of patients between 27-30 plus at least 1 weight related condition. They were then divided amongst various groups that received 1 mg, 4 mg, 8 mg, and 12 mg weekly injections. At 48 weeks (about 11 months), there was approximately 9% reduction in weight in the 1 mg group, 17% reduction weight in the 4 mg group, 23% reduction weight in the 8 mg group, and 24% reduction weight and 12 mg group compared to 2.1% reduction in weight in the placebo group.

In regard to the side effect profile, the most common side effect was nausea which went up significantly at the 8 and 12 mg group but a third less in the folks at the 4 mg group.

In regard to the Tirzepatide trial, people assigned to the 5 mg/week, 10 mg/week, and 15 mg/week. At 72 weeks, there is approximately 15% reduction, 19.5% reduction, and 21% reduction, respectively. There is approximately 3.1% reduction in the placebo group.

Interestingly, GI side effects with nausea were remarkably more in patients taking tirzepatide compared to retatrutide. Tolerance of the retatrutide will lead to more sustained weight loss.

Based on the data, retatrutide will be greatly beneficial for patients who are intolerant to tirzepatide and or not having the desired reduction in weight and improvement in metabolic health on it. Clearly the data shows that the primary weight loss is seen in patients who are taking 8 to 12 mg of retatrutide.

So, from a personal practice point of view, we recommend starting a patient on tirzepatide, but if we see that patients are not tolerating it, or if they don’t have the desired reduction at higher doses, then we will switch them to retatrutide at least 8 mg/week would be the right approach.
